Why does my visitor’s status says “Failed”?

There are two reasons why a visitor’s status can be labeled Failed

1.  The visitor selected “Yes” as an answer in the site questionnaire (COVID-19 questionnaire). 
   OR
2. A screener selected “Fail” after screening the visitor.
